Post-Concussion Symptom Test
in Calabasas, CA
Weeks after a head injury and still not yourself? Start by mapping what has changed.
The Rivermead Post-Concussion Symptoms Questionnaire (RPQ) is one of the most widely used tools for tracking symptoms after a concussion or head injury. Dr. Alireza Chizari, DC, DACNB, a board-certified chiropractic neurologist, may use results like these to understand which symptom areas are most affected before an evaluation.
- 16 symptoms
- About 2 minutes
- Score 0–64
What the RPQ measures
The RPQ asks about 16 common post-concussion symptoms, each rated 0–4 compared with before the injury. A rating of 1 (“no more of a problem”) is scored as zero, so the total runs from 0 to 64. Many clinicians also look at RPQ-3 (headache, dizziness, nausea) and RPQ-13 separately.
Why your score matters
Post-concussion symptoms often cluster — dizziness with visual strain, or fatigue with poor concentration. Seeing which areas are highest can help a clinician decide where to look first, and repeating the RPQ shows how symptoms change over time.
Other Symptom Questionnaires
Symptoms often overlap. These short questionnaires can help you describe the full picture before your visit.
Frequently Asked Questions
How long do post-concussion symptoms usually last?
Many people feel better within a few weeks, but some have symptoms for months. Everyone's recovery is different, and lingering symptoms are a good reason to seek an evaluation rather than wait.
What is a high Rivermead score?
The RPQ has no official cut-off. Higher totals mean more symptoms compared with before the injury. Some clinics use 13–24 as mild, 25–32 as moderate and 33 or more as severe as a general guide.
What are RPQ-3 and RPQ-13?
Research suggests the first three items (headache, dizziness and nausea) behave a little differently from the other thirteen, so they're often reported separately as RPQ-3 and RPQ-13.
Is my information saved or sent anywhere?
No. Your answers are scored in your own browser and aren't stored or sent to the clinic. You can print or save your results as a PDF.
