Frequently Asked Questions
Which self-assessment should I take first?
Start with the one that matches the symptom bothering you most — use the symptom picker above if you’re not sure. Many people’s symptoms overlap, so it’s common to take two. For example, after a concussion both the post-concussion and dizziness questionnaires can be useful.
Are these online tests a diagnosis?
No. Each one is a validated questionnaire that describes how symptoms affect your life. They can’t identify a cause or diagnose a condition — that requires an in-person evaluation with a licensed clinician.
Is my information private?
Yes. Every questionnaire is scored inside your own browser. Your answers are not stored or sent to California Brain & Spine Center or anyone else. You can print your result or save it as a PDF.
How long do the self-assessments take?
Between about one and five minutes each. The Headache Impact Test is the shortest at six questions; the autonomic symptom questionnaire is the longest, though its built-in skip logic often shortens it.
What should I do with my results in Calabasas?
Bring them to your visit. At our Calabasas clinic, Dr. Chizari reviews questionnaire results alongside a full history and neurological examination to help decide whether functional neurology care is appropriate for you, or whether a referral to your primary care provider or a medical neurologist is the better step.
